Liability Waiver, Express Assumption of Risk, and Release of Liability
Mindfulmess Mom provides an online and in-person forum for discussion where parents can share thoughts, ideas, and concerns related to their parenting experiences. The classes and coaching services provided by Mindfulness Mom are provided by a non Licensed Therapist; however, these services are not intended to be a substitute for the advice and/or treatment by your health care providers; nor are these services to be received as mental health treatment. Furthermore, the needs of each child and parent are unique, and no service or forum provided by Mindfulmess Mom is intended to replace the advice of a healthcare and/or childcare professional.
You further confirm that:
- You are the parent and/or lawful guardian of the child or Teacher.
- ACTING ON BEHALF OF YOURSELF, AND TO THE EXTENT ALLOWED BY LAW, YOU HEREBY WAIVE AND AGREE TO RELEASE ANY CLAIMS OR LIABILITY WHICH YOU, YOUR CHILD, OR YOUR CHILD’S HEIRS AND SUCCESSORS MAY HAVE OR BELIEVE TO HAVE AGAINST Conscious Mommy, LLC AND ITS OFFICERS, EMPLOYEES, OR AGENTS FOR ANY INJURIES, LOSSES OR DAMAGES TO YOUR CHILD, YOUR CHILD’S PERSONAL PROPERTY AND YOUR PERSONAL PROPERTY.
- YOU AGREE TO BE RESPONSIBLE FOR, INDEMNIFY, AND HOLD HARMLESS Conscious Mommy, LLC FROM AND AGAINST ANY CLAIMS, SUITS, JUDGMENTS, OR COSTS WHICH MAY BE BROUGHT AGAINST Conscious Mommy, LLC, ITS OFFICERS, DIRECTORS, EMPLOYEES OR AGENTS FOR THE ACTUAL OR ALLEGED ACTS OR OMISSIONS OF YOU OR YOUR CHILD.
I have read the above Liability Waiver, Express Assumption of Risk, and Release of Liability in its entirety and I freely and voluntarily assume all risks of such Injuries and Damages and notwithstanding such risks, I agree to participate in the services and workshops provided by Mindfulmess Mom.
